Payment/scheduling/insurance: 

• Payment is always collected at the time of service (cash, check or CC)

• My fee schedule:
  $400 for initial visit/full psychiatric evaluation, 75min.
  $200 for medication management follow-up visits, 30min.                $300 for therapy + medication management follow-up visits, 45min.
• I will provide a statement that you can submit to your insurance company.  I am considered an out-of-network provider for most insurance companies.  Many insurance policies do provide some out-of-network reimbursement for mental health services.  Additionally you can always utilize your Health Savings Account (HSA) or Flex Spending Account (FSA).
